An off-duty Ontario Police Department officer died in a solo vehicle collision in San Bernardino on Nov. 2, according to the San Bernardino Police Department.

The deceased officer was identified as Joshua Ray Burks, 40, who was driving an unmarked Ontario city vehicle, police said.

At about 12:14 a.m., Burks was traveling north on University Parkway, approaching Kendall Drive, in a sport utility vehicle. The driver lost control and the SUV collided with a tree in the center median. As a result of the collision, the injured driver was transported to an area hospital. The driver, who was the only occupant of the vehicle, eventually succumbed to his injuries and died at the hospital.

At this point, the cause of this traffic collision is under investigation. Anyone with additional information is asked to contact Sergeant Equino Thomas or Sergeant Jeff Harvey at (909) 384-5792.
